What To Expect At Your First Visit
Your first visit to Lost Mountain Dental is important because it establishes a vital foundation for your dental care going forward. During this first visit, we'll need to get some important background information, including your medical history, and you will have plenty of time to get to know Dr. Mulkey.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide you with the best possible dental care. It will also help relieve any unnecessary anxiety you may be feeling.  You'll find all the New Patient Forms that will need to be completed below. If you'd like, you can print them out at home or work and bring them with you when you come for your appointment.

Also, to help ensure your first visit goes as smoothly as possible, we invite you to take a few minutes to visit our Patient Education section to educate yourself about the  symptoms you're experiencing or to learn about some of the cosmetic dentistry smile solution we offer.
